Summary Care Record

What is Summary Care Record (SCR)?

The Summary Care Record (SCR) is a national scheme and is used to support your emergency care. It will contain important information about medicines you are taking, any adverse reactions you have had to medications, and any allergies you have.

Giving healthcare staff access to this information can prevent mistakes being made when caring for you in an emergency or when your GP practice is closed.

As a patient you have a choice:

* You DO NOT need to do anything if you would like a summary care record created for you.

* If you wish to opt out of the Summary Care Record programme please ask either your GP or reception staff for an opt out form - at anytime you are able to change your mind, just let your GP surgery know.
